Advance Care Planning (Nurses, CCM, CRC, and NASW approved) – 3.0 CE/Contact Hours

Course Objectives

By the end of this course, the learner will be able to:

• Recognize and describe the potential barriers for people in planning for their future in both matters of personal assets and healthcare decision making.
• Identify communication techniques to help facilitate future care planning discussions with patients and their family members.
• Define the purpose and process for executing advance directives and identify the resources and tools available to assist patients in facilitating this process.

Continuing Education Credits

This course is approved for 3.0 CE/Contact Hours for the following professions:

• Nurses (RN, LVN, LPN) (all states)
• Certified Case Managers (CCM)
• Certified Rehabilitation Counselors (CRC)
• Social Workers (NASW)

Course Details

To receive 3.0 contact hours of continuing education credit, learners must pass the final quiz with 80% or more; the final quiz may be re-taken as often as needed.
