DESCRIPTION: This course examines the distinct clinical trajectories individuals may follow at the end of life—from sudden, unexpected death to gradual decline due to chronic illness or frailty. Participants will explore the implications of each pathway, learn to recognize the evolving needs of patients and families, and apply evidence-based interventions to deliver compassionate, person-centered care. Through this course, participants will be better equipped to support patients and families during one of life’s most vulnerable transitions.

  1. Identify and differentiate between three major clinical trajectories at end of life
    • Sudden death
    • Prolonged Decline/Frailty
    • Terminal Illness
  2. Discuss common physical, emotional, and psychosocial patterns within each trajectory.
  3. Explain evidence-based, person-centered interventions to support patient, family, team and self needs across the diverse pathways of dying.

Presenter Spotlight

Lee-Anne Godfrey is a Registered Nurse with a Bachelor of Science in Nursing from the University of British Columbia. She is also a Certified Care Manager. After working in various clinical settings, including Acute Medicine and Hematology/Oncology, Lee-Anne worked as an Education Specialist for the Alzheimer’s Association, a hospice Nurse Liaison, and Director of Quality Assurance and Education in the Home Care industry. 

In 2014, Lee-Anne founded a Nurse Care Management company, RN Health Management, in Orange County, CA. With a focus on the older adult, the company specializes in complex medical and healthcare situations ranging from acute illness to chronic conditions. Registered Nurses, along with a Licensed Clinical Social Worker, manage, navigate, monitor, and coordinate care for their clients.
