DESCRIPTION
This course provides an advanced overview of advance care planning. By the end of this course, learners will be able to:
- Identify and describe the potential barriers for people in planning for their future in both matters of personal assets and healthcare decision making.
- Identify communication techniques to help facilitate future care planning discussions with patients and their family members.
- Define the purpose and process for executing advance directives and identify the resources and tools available to assist patients in facilitating this process.
Approved for 3.0 CE/contact hours for:
- Nurses (all states)
- Certified Case Managers (CCM)
- Certified Rehabilitation Specialists (CRC)
- Social Workers (NASW).
Must pass final quiz with 80% or more; may re-take as often as needed. Course will be updated or discontinued on or before July 31. 2027.
