DESCRIPTION
This course provides an advanced overview of the Hospice Model of Care. It is appropriate for healthcare workers, social service professionals, and other professionals who may interface with hospice, as well as for patients, families, and others who are interested in learning more about hospice services and philosophies. Learners will achieve the following objectives:
- Describe the philosophy of Hospice Care.
- Identify five core members of the Hospice team and each role.
- Explain three components of Hospice Care.
- Define eligibility criteria for Hospice.
- Analyze a case study involving Hospice.
Course is approved for 4.5 CE/Contact Hours to the following professions:
- Nurses (RN, LVN, LPN) (all states)
- Critical Care RN (CCRN): meets the criteria for CCRN Synergy Continuing Education Recognition Point(s) (CERPS) in Category B
- Certified Case Managers (CCM)
- Certified Rehab Counselors (CRC)
- Social Workers (NASW)
This course is also approved for 4.5 ethics hours for Nurses, CCM, and CRC. Must pass final quiz with 80% or more; may re-take as often as needed. Course will be updated or discontinued on or before July 31, 2027.
