DESCRIPTION
Course Objectives
By the end of this course, the learner will be able to:
- Define multidisciplinary care and its importance in managing chronic diseases.
- Identify the key roles of different healthcare professionals in a multidisciplinary team.
- Develop effective communication strategies for coordinating care among team members.
- Facilitate collaborative decision-making processes that involve all relevant stakeholders.
- Explore the role of Health Information Technology (HIT) in supporting multidisciplinary care by analyzing patient data to inform care plans and track outcomes across different specialties.
- Assess the effectiveness of multidisciplinary care models in managing chronic diseases.
- Implement continuous improvement strategies to optimize team performance and patient outcomes.
Continuing Education Credits
This course is approved for 1.0 CE/Contact Hour for the following professions:
- Nurses (RN, LVN, LPN) (all states)
- Certified Case Managers (CCM)
- Certified Rehabilitation Counselors (CRC)
- CA Workers’ Compensation Claims Adjusters (WC CA)
Course Details
To receive 1.0 contact hour of continuing education credit, learners must pass the final quiz with 80% or more; the final quiz may be re-taken as often as needed.
